On this page you'll find all the paperwork and forms you will need to audition for America's Got Talent

AUDITION INFORMATION FORM
Everyone who auditions will need to read and sign our Audition Information and Personal Release Form. Download the forms now to get a jumpstart on the audition process.
Please bring these forms with you along with a picture ID.
PHOTO I.D.
EVERYONE will be required to present a photo ID in order to attend the audition, including friends and family.
Minors CONTESTANTS AND GUESTS without a driver's license may use a school ID
or acceptable substitute (such as a yearbook page that includes a printed name with photo
AND a birth certificate matching the name). If the minor is too young to have any form
of photo ID, then that minor's parent or guardian should present a photo ID and the
child's birth certificate that includes the parent's name with the child's name on the
certificate.
FRIENDS AND FAMILY
We encourage you are bringing friends and family members to support you. Just keep in mind they will not be allowed in the actually audition room (unless it is a minor who is auditioning). All your supporters will need to download this release form, sign the bottom, and bring it to the audition.
IMPORTANT MINORS INFORMATION
EVERYONE under 18, contestants or supporters, MUST have permission from their parents or legal guardian to audition AND must be accompanied by one of their parents or their legal guardian.
All minors attending the audition will be required to present a photo ID. Minor contestants may use a school ID or acceptable substitute (such as a yearbook page that includes a printed name with a photo AND a birth certificate). If the contestant is too young to have a photo ID, then the parent or guardian should present a photo ID & the child’s birth certificate that includes the parents name with the child’s name on the certificate.
If the parent or legal guardian of the under 18 minor is unable to attend, and is allowing the minor to attend the audition under the supervision of a third-party (such as a coach), you MUST do two things:
2) The parent or guardian must designate the third-party specifically on the Personal Release, with a special signature by the parents or legal guardian and the third-party (see page 8 of the Personal Release).
ANIMAL ACTS
If your act includes any type of animal you will need two sets of paperwork with you at your audition:
1. You will need a copy of your inoculation records from your veterinarian, verifying that your animal(s) have all of their current vaccinations.
